Flexible Support At Home

We offer flexible support services to enhance the quality of life of seniors and their carers living at home. Sometimes it is the little things that make a big difference - such as help with cleaning, showering, dressing, or much-needed respite.

As these services are partly Government-funded, they are available to seniors and their carers who have been assessed as eligible to receive a Community Aged Care Package (CACP) by an Aged Care Assessment Team. This is designed for people whose needs can be managed at home, with support.

Our CACP Case Manager uses a consultative and holistic approach to develop a care plan and to coordinate appropriate services, which may include:

  • Home care - house cleaning, laundry, meals preparation
  • Personal care - showering, dressing, hygiene
  • Assistance to access social and recreational activities and day programs
  • Short-term in-home respite for a carer
  • Minor home and garden maintenance
  • Assistance to access community transport
  • Assistance to access allied health services, such as podiatry, occupational therapy, etc.
  • After hours support services, if required
  • Referral to other services.

Our Case Manager monitors and reviews services on a regular basis to ensure they are meeting the identified needs.

Catholic Homes currently provides these services to eligible seniors living in the following cities:

Boroondara, Brimbank, Darebin, Hobson's Bay, Hume, Knox, Manningham, Maribyrnong, Maroondah, Melbourne, Monash, Moonee Valley, Moreland, Nillumbik, Whitehorse, Whittlesea
Wyndham, Yarra, The Yarra Ranges.
